I think such tough choices will have to be decided by the patient before not too long in the form of more taxation and or insurance

Easy to say that when you're fit as a premier league footballer with a resting pulse under 50, who can run sub 6 minutes miles over 5k, swim a mile in under 30 minutes, with no problems doing 100 press ups and 25 pullups, win triathlons, ability to do all sorts of gymnastic stunts, have a bodyfat percentage below 12%, army and combat/martial arts trained and can "handle" themselves better than most and only 39 years old living a fantastically glorious outdoor pursuits lifestyle wind surfing instead of playing computer games.

So if you saw one of your fellow surfers in mortal danger of drowning you'd be happy stand by and not offer help or at least call someone who can and just insist "oh well tough ****, shouldn't do such a risky sport."?

What if one was subject to abuse as a child or had mental health problems? Would they then be candidates for neglect by society or would they be worthy of help and support and treatment? If one's parents were alcoholics and smokers and gamblers or drug addicts should they be left to self-destruct owing to their 'personality flaw'? What about babies that are born addicted to heroin because their mothers were jacking-up during pregnancy? Flawed, are they?

I don't have all (if any) of the answers but if any person creates a persistent drain on resources through either issue, I think help should be stopped.
Kids born addicted, kids brought up in severely negative environments are a different matter.
Everyone should be given a chance but persist in blowing that chance and you should be shown the "support" door.
