Think it was Saw 2, where the druggy women had to jump into a pit full of hypodermic needles/syringes and try and quickly find a key to realese herself by digging into them as fast as she could
I cant stand needles at all, and that seen had me cringing BIGTIME

Eden Lake, when they set fire to them people alive. the little asian kid with the tyre round his neck alight squeeling away, horrible scene.
All the other obvious ones mentioned above, the baseball bat scene in Casino was horrid, as was America History X teeth+kerb scene

"Kee kee kee kee" - haha I was like "WTF?!" and had to rewind that scene to watch it again.. yes a big time horror movie fan here.
Also "Excorcism of Emily Rose" - something about that film is very very disturbing, horror movies don't phase me normally but this movie is just down right creepy because its based on real life events.
The one scene would be when the brother wakes up Emily's dorm room and finds her all twisted up on the floor!

Will agree with a lot of these so no point reposting those but to add a couple more:
When i was wee it was American Werewolf in London, great film but specifically the bit in the woods when she wakes him in his hospital bed. Blimey!
Also Japanese film called Ichi The Killer, lots of rough bits in that but specifically a section involving some very hot fat and some bare flesh...

I'm making a mental note not to watch any more Japanese films....I remember watching one as a child (elder brother was babysitting and he was a fiend for putting horror films on) loads of blood, decapitations etc. I seem to recall a toddler in a pram with blades sticking out of the wheels, possibly Shogun Assassin?

Can't remember the name of the film, but has a woman walking down a line of her most loyal soldiers (Centurions). She picks a couple of them to show their loyalty to her by sliding down a slide which is basically a huge blade, which cuts them in half as they slide down with a leg either side of the blade. Cross legged moment!
The scene from "We were Soldiers" where two guys pick up one of their own troops badly burned by Napalm and the skin and flesh on the guys legs comes off.

the scene in Mirrors when the ghost gets his sister to pull apart her mouth until in splits : puke: it's the sort of scene you think will stop about 10 seconds before it actually does, you can't quite believe they've gone so far.
a bit like the burning of the poilce woman in Silent Hill, that went on for a bit too long. burning is one thing but when you see the blisters appearing on her skin and popping. yuk.
