It's all about following process and staff understanding the importance of doing so, assuming a decent framework and underpinning strategy has been defined then things like this just should not happen. Human error should not even come into it if processes are in place to both protect the staff and services.
No need for over complex and time intensive processes (these are just as bad as a lack of process) but having best AND good practice frameworks in place can save a company.
Basically a massive messup that will originate from complacency or ignorance at the top and has landed on the little man in ops' shoulders.
Easy to solve! Difficult for some to get their heads around.

I can easily envisage how this would have happened, and the feeling he/she must have had in the seconds aftewards, because you would know immediately the impact of it.

Without getting into specifics, no amount of process or framework could have prevented this. The process of backing out this particular thing means you have to make some decisions and take some manual actions. It is open to error, and unfortunately, they made one.
I can easily envisage how this would have happened, and the feeling he/she must have had in the seconds aftewards, because you would know immediately the impact of it.

If users/operators etc. understand and follow process correctly, with other processes in place to protect and back up actions taken, then mistakes like this wouldn't happen. But I'll agree to disagree

Wages aint in, apparently if i go to the back with my pay slip they will give me cash, fat load of use because I'M IN WORK ALL DAY! hopefully they are open this weekend so I can pay the rent.
