I use a WinFast TV2000XP Deluxe card. Works very well.
Captures in MPEG-1, 2, 4 or any other codec you fancy.
If you don't want to do it yourself there of loads of places that will convert it for you with chapters, menus and stuff at a rate of about £25-30 per hour of footage.
