PC Slowed Down ..... ?

My PC has become really slow ..... at first I thought in was the Internet connection. A Speed Test gives me Download at 240kbps and Upload at 318kbps and, although not great, I don't think it is any worse than it has always been?
It seems to be the PC itself. Sometimes as I type the letters don't appear - like the CPU is busy elsewhere (but everything is turned off) it's like my very first PC 12 years ago when the plotter was working ... every slowed to a snails pace.
I am at a loss as to what to do, done viruse checks, clean ups, disk cleanup, defrag .... turned off most of the Start-Up programs - it's pathetic.
Only happened in the last week or so .... any ideas?

Spec. is low - it's about 7 or 8 years old.
I have done Ctrl Alt Del previously when it slows down ... it shows CPU usage at 100% - even though I cannot see anything running which shouldn't be running, then again - does anyone understand what all those running items in the list actually mean? I don't!

you should see in the processes tab which program is using the most CPU, just watch it for 5 mins, you will see it eventually
click on the most one being used (might be antivirus updating), and end process
