just give mine a quick wrap, i think it is best to wrap each pipe like above, but i didnt have the time do to do it that way.
i used stainless steel locking wire to hold it all in place

it can be, but it is alot easyer if you wet the wrap. it takes longer to wrap each pipe on there own, but works better.
it took we about 1.5 hours to wrap them and lots of locking wire in each pipe

also it is worth buying good hear wrap, not the cheap rubish off ebay, i had the cheap stuff 1st, and it falls apart fromthe heat etc fairly quickly.
i have got this stuff now. you can buy it from zen performance, i think i used around 1 and a half rolls, on my headers and up pipe, but i think you can get away with 1 roll if you are carefull.
i also used 1 roll on my down pipe
Thermal Logic 'Extreme' insulating exhaust wrap (2" x 50ft) £58.75 +vat
(at the bottom)

Here is a pic of mine GT spec 2's when being fitted fitted, used lock wire instead of the crap straps for a much better job,

Is this purely a cosmetic mod to look nice or does wrapping headers increase efficiency / performance in any way?

Keeps heat out the engine bay, which is a good thing.

how many layers do you put around the pipes?

1 on each pipe then wrapped all together,mite do another layer tomoz

Careful with to many layers, DEI only recomends a 2-3mm overlap on the wrap. Could get to hot under there i guess!
