Did you photocopy the original V5 (all sides)? Doc ref number is on that. Re-apply for sorn after getting the new certificate too.
Helpful note: Always photocopy EVERYTHING you send to the DVLA (including cheques preferable with the return form behind it with reference details visible) - trust me on this; the amount of stuff they "lose in the post" is unbelievable - and they'll blame you for non-reciept of anything due to them.
