Default Small end bearing failure

Hi guys,
question for all you, my 00GT (Impreza Turbo) has had a small end bearing failure, explain this to me. What would cause it ( Launch control, bigger turbo, 95 octane fuel??)Dastek Unichip. Car has done 130 000km. Should the tuner have some responsibility, after fitting launch control we found a shimm on the exhaust cam at a angle rather than flat, lookied like it was trying to come out, cause of launch control??

Any help would be appreciated

My experience of small end bearing failure in a 900 honda 16 valve F1 engine, is using excesive high rev's. Nothing at all to do with oil supply, just high rev's. I bet if you inspect them they have e-longated, typical problem which starts with a slight rattle on over run, then as time goes by the rattle becomes more permanent, then bang...
