If you can be bothered, do the lot.
I couldnt, if i want to listen to good quality music, its in the house, thats where av spent my money.
Been and done the car sterreo thing and it always ends up being changed, and there fore never ending.
Changing the speakers, you will need new spacers for the front, there are some good quality ones that can be bought rather than having to cut your own from MDF.
Head units, well, thats pretty mush up to what you want/need, and how far your going to take the system.
search the ICE forum for details of what other have done
