Very, very simple. Wheel off, undo one of the bolts holding the calliper to the hub and slacken the other and the calliper should rotate away from the disc. Exchange discs. Replace calliper bolts, well.
Remove the two retaining pins on the top of the calliper, slide out the long pins and the pull out the pads and replace using copper grease on the backs of the pads. You may need to push the pistons back into the callipers and you may lose some brake fluid out of the top of your reservoir if the difference in pad thickness is substantial.
Replace pins and clips, bed in as per pad instructions.

Recommended caliper bolt torque 75 ft-lb. Figure from Alyn, manual says 129 ft-lb and seems an awful lot. When I did mine it turned sour as one of the caliper bolts was damaged from new and damaged the thread in the caliper (out of warranty - thanks Subaru).
